Adhesive capsulitis, better known as frozen shoulder, is a condition in which there is a significant reduction in shoulder movement due to inflammation of the soft tissue shrinking around one of the shoulder joints. The shoulder girdle is made up of a combination of three joints: the glenohumeral, acromioclavicular and scapulothoracic articulations.
The glenohumeral joint (the ball and socket joint) is the joint most people are familiar with when they think of the shoulder, and this is the joint which is involved in frozen shoulder. Adhesive capsulitis can occur following a traumatic event and subsequent immobilisation, or it can develop gradually with no identifiable cause.
The Three Stages of Adhesive Capsulitis
- Stage One – Freezing: Predominantly pain is experienced or increases throughout this stage as the shoulder gradually gets stiffer; this can last between 3-6 months.
- Stage Two – Frozen: Pain starts to reduce during this stage, but the stiffness stays usually between 4-12 months.
- Stage Three – Thawing or defrosting: During the third stage, the stiffness gradually decreases and the range of movement starts to return, lasting between 12 to 24 months.

A Multidisciplinary Approach to Care

In some cases, an ultrasound or X-ray may be useful to exclude other pathologies that can often predispose a shoulder to become frozen. Occasionally a cortisone injection can help if there is an inflamed bursa or tendon.
